Your First Visit

A clear starting point. A plan built around you.

Whether you want to run, golf, work out, or move more comfortably, your first visit starts with understanding what’s holding you back—and what you want to get back to.

At Peak Performance Sports Therapy, we take time to listen, assess your concerns, and explain your options so you can feel confident about your next steps.

Before You Arrive

Complete your intake forms
Please complete the forms sent with your appointment information. Your health history, symptoms, and goals help us make the most of your time.

Bring relevant information
If available, bring imaging reports, surgical details, or recommendations from your healthcare provider. You don’t need to obtain new imaging just for your appointment.

Wear comfortable clothing
Choose clothing that lets you move freely and provides access to the area being assessed. Shorts work well for lower-body concerns; a tank top or athletic shirt can help with shoulder and upper-body assessments.

If you’re receiving hands-on treatment, please avoid applying lotions or oils to the treatment area beforehand.

What to Expect

We’ll Start With Your Story

We’ll discuss your symptoms, previous treatment, daily activities, and the things you’re having trouble doing. Just as importantly, we’ll talk about what successful recovery looks like for you.

We’ll Assess What Matters

Your assessment will depend on your concerns and the service you’ve booked. For physical therapy, this may include movement, strength, mobility, and tests related to your symptoms.

We’ll explain what we find and how it guides our recommendations.

We’ll Build Your Plan Together

Your care may include hands-on treatment, exercise, or technologies such as shockwave or laser therapy when appropriate. We’ll discuss the purpose of each recommendation and what you can do between visits.

Treatment should feel manageable. Tell us if anything feels too intense or uncomfortable so we can adjust.

After Your Visit

You’ll leave with clear next steps, which may include exercises, activity adjustments, and a recommended follow-up schedule.

Some treatments can cause temporary soreness. We’ll explain what to expect based on the care you receive. Contact us if your symptoms are worsening, feel unexpected, or leave you unsure about what to do.

As you progress, we’ll reassess and adjust your plan around your response and goals.

Care That Keeps You Involved

You deserve to understand your care and feel heard throughout the process. We welcome questions, respect your comfort, and work with you to make treatment practical for your life.
